アメリカの病院からセカンドオピニオンを(MSKその２)
MSKより早速返信が来ました。
必要な情報を送ってくださいと言う内容で、そのリストが書かれていました。
- Photocopy of the patient’s passport
- A recent medical summary from the patient’s physician detailing medical history, current condition, and treatment
- All medical reports related to your diagnosis in English
- Radiology reports from the past 3-6 months
- Laboratory reports within 14 days
- All treatment records (chemotherapy and radiation therapy)
- All operative reports and discharge summaries
- A copy of the original pathology report (in the original language) and an English Translation (both the original and the English translation of the pathology report must include the following: patient name, date of birth, specimen number, date of the procedure, and hospital name/address where the procedure was completed)
- Cover letter with questions (optional – not all questions may be appropriate to answer remotely)
- Fee for remotes services: Please see attached Description of Remote Services. Once we have received your medical records and are prepared to move forward, a link will be provided to you to process the credit card payment online through US bank.
- Materials required for Pathology Review:
- Pathology slides (stained), 10-15 unstained pathology slides or wax paraffin block(s), and a copy of the original reports and the translation in English.
- When submitting pathology specimens (tissue samples), please make sure to send a copy of the original pathology report and an English translation. Original and English versions of the pathology reports must have the following items listed:
- Patient’s name
- Patient’s date of birth
- Date of surgery
- Specimen Number
- Specimen part type
- Hospital/Laboratory name and return address for material
- Materials required for Radiology Review: (2 max. reports issued)
- Radiology images on a DICOM formatted CD
- Last two recent Radiology Images per modality (EX: the last two images of CT’s, MR’s, US, X-rays and/or PET’s) with corresponding reports in English (All Radiology Materials Must be in DICOM format or the films) Please make sure to identify on each CD the type of test performed, body part and the date(mo抄訳nth/date/year) when the test was performed.

↑なぜか8.でなく1.なのは多分間違いだと思う
ちょっと分かりづらい部分はあるけど、ひとまず県立がんセンターの先生にお願いして診療情報提供書をお願いしました。
通常の国内のセカンドオピニオンの場合と同じ内容の書類を用意して頂き、それをプロに翻訳してもらうつもりです。
それから添付ファイルとしてリモートサービスの料金やオプションなどが書かれたPDFファイルが来ていました。

高度な病理レビュー(必要な場合)
それ以外にも、独自の遺伝子スクリーニング検査や、Integrative Medicine Consultと言う統合医療（具体的にはエクササイズや食事、鍼やマッサージなどのアドバイス）などのオプションが有るようです。
僕はが受けてみたいのは、標準的なリモートセカンドオピニオン、または単独の病理レビューです。
また、面白いと思ったのはIntegrative Medicine Consultと言う統合医療相談。日本では民間療法に分類される鍼治療なども選択肢に入るようです。最新の西洋医学的視点で代替医療の様な治療法を取り入れるアドバイスがもらえるかも知れない。おそらく日本では制度的な理由もあり、切り離して考えざるを得ないと思う。好みによるとは思うけど、鍼治療などを積極的に取り入れたいと考える人には価値の有る選択肢かも知れない。
